By submitting this request for inclusion in the TINRC Provider Directory, I hereby attest and affirm that all of the information I have provided is truthful and accurate. I further attest and affirm that the services provided are done in a way that is in compliance with trauma informed best practices and that if any of the information that I have provided changes at any point, it is my responsibility to timely inform TINRC of such change(s) so that the Provider Directory can be updated. I further understand that, if at any time, TINRC determines that I should not be included in the Provider Directory for any reason, TINRC shall have the right to remove me from the Provider Directory.
