SOMOS Community CAre

SOMOS Community Care (SOMOS) is a network of physicians in New York operating in underserved, economically disadvantaged communities throughout New York City as well as in Nassau and Westchester counties. SOMOS works to increase the quality of care and life of the communities it serves by integrating behavioral and social needs into the wellbeing of the person and the family. Along with addressing and integrating social needs, the Primary Care and Behavioral Health Integration Project was selected as SOMOS’ primary project. The goal of this project is to integrate primary care practices with behavioral health. By doing this, we hope to raise awareness through education, reduce hospitalization and stigma, and improve the standard of care.

In alignment with this mission, SOMOS serves as the lead organization of the Bronx Social Care Network (SCN)—a collaborative network of hundreds of community-based organizations and providers that connect patients to essential services such as housing, nutrition, employment, transportation, and legal support. This integrated infrastructure allows SOMOS to address the social determinants of health that often underlie medical and behavioral conditions. Moreover, all initiatives are grounded in a trauma-informed care framework, ensuring that every level of service delivery—clinical, operational, and community-based—is guided by safety, trust, empowerment, collaboration, and cultural responsiveness. Together, the SCN and trauma-informed principles strengthen SOMOS’ holistic approach to improving community health outcomes and promoting healing across systems of care.
